Insurance

Please use the chart below to check that your psychiatrist or therapist is credentialed with your insurance plan prior to scheduling.

OPM provides courtesy insurance verification for all of our patients. You are welcome to contact our office for an estimate of your expected charges. We do our very best to get accurate information from insurance companies, although the information they give us is not always 100% correct. It is ultimately your responsibility as a patient to be familiar with your insurance coverage and to check your insurance handbook.

We require a valid credit or debit card to be stored on file. After your appointment, we will submit a claim to your insurance company first. After your insurance company processes the claim and issues an Explanation of Benefits (EOB), an invoice for any remaining deductible, coinsurance, copay, or other patient responsibility will be emailed to you. After 7 days, if you have not paid the invoice using another payment method, the amount will be charged to the card on file.

BlueCross BlueShield BlueCard

If you have a BCBS plan from any state while living in Oregon or Idaho and your insurance card includes the BlueCard® logo, we are often able to accept those plans, although coverage may vary by policy. BridgeSpan

Our providers are in-network with all BridgeSpan plans, including the Gold, Silver, and Bronze tiers of the Individual Value and RealValue plans.

FEP Blue (Federal Employee Program)

We are in-network with FEP Blue (Federal Employee Program) through the Blue Cross Blue Shield Preferred Network. If you have a FEP Blue plan, your appointments are likely covered at your in-network benefits. For more details, please contact us or visit our FEP Blue insurance page.

Health Net of Oregon

Dr. Christopher Galbick, DO is in-network with Health Net of Oregon. Patients with eligible Health Net commercial plans may be able to receive psychiatric evaluation, medication management, and telehealth services using their in-network behavioral health benefits. Healthcare Management Administrators

Oregon Psychiatric Medicine participates with Healthcare Management Administrators (HMA) for many employer-sponsored health plans that include behavioral health coverage. Patients whose insurance cards list HMA as the administrator may be eligible to receive psychiatric evaluations, medication management, and therapy services through our practice while using their in-network benefits. Moda Health (Connexus and Synergy Plans)

Our practice is in-network with Moda Connexus and Moda Synergy plans. Patients with these Moda Health commercial plans may be able to receive psychiatric evaluations, medication management, and therapy services. MultiPlan / PHCS

If you have a PPO plan with another insurance provider, check your insurance card to see whether it includes a MultiPlan® or PHCS® logo. Many employer-sponsored plans use the MultiPlan / PHCS network to provide access to a broader group of participating providers. Our clinicians participate in the MultiPlan / PHCS network, and patients whose plans include this network may be eligible to use their in-network benefits. PacificSource

Dr. Christopher Galbick, DO is in-network with PacificSource Core, Navigator, and Voyager plans. We are not able to accept Legacy, Blue High Performance Network, or Medicare Advantage plans.
