As we transition to accepting commercial insurance, we are now requiring a valid card on file for all patients.
You are welcome to contact our office before your appointment for an estimate of your expected charges. We will submit the claim to your insurance first. After your insurer processes the claim and issues an Explanation of Benefits (EOB), any remaining deductible, coinsurance, copay, or patient responsibility will be charged to the card on file.
For patients with Medicaid/OHP, the card will not be charged.
